A patient comes to the emergency department with a red, swollen leg and pain that seems out of step with how the leg looks. The area is warm and tender, maybe a little firm, but there is no obvious wound and no dramatic skin change. The pain, though, is severe, and it is getting worse fast. They are diagnosed with cellulitis, given antibiotics by mouth, and sent home — or admitted to a medical floor for IV antibiotics and observation. Hours later the pain is unbearable, the skin has darkened and blistered, the blood pressure is falling, and an emergency operation finds that the infection has been racing along the tissue beneath the skin the entire time. The diagnosis is necrotizing fasciitis, and by the time the operating room is reached, the patient has lost a limb, or worse.
Necrotizing fasciitis — the “flesh-eating” infection — is one of the hardest emergencies in medicine to catch early and one of the most punishing to catch late. Its defining trap is that the skin surface lags behind the destruction underneath, so in the first hours it can be indistinguishable from an ordinary skin infection. This guide explains what separates a genuinely difficult case from a diagnostic error, the warning signs and workup the standard of care is built around, why the surgical clock matters so much, why a “reassuring” lab score can be a trap, and the records that decide these cases.
What Necrotizing Fasciitis Is
Necrotizing fasciitis is a rapidly progressive bacterial infection of the fascia — the sheet of connective tissue beneath the skin and around the muscles — and the surrounding soft tissue. The bacteria spread along these tissue planes and release toxins that kill the tissue and clot off its blood supply, which is why the destruction can outpace the skin changes visible on the surface. According to the U.S. Centers for Disease Control and Prevention, group A Streptococcus is one of the most common causes, although many infections are polymicrobial, and it can begin from something as small as a minor cut, a surgical wound, an injection site, or even blunt trauma with no break in the skin.
The reason speed is non-negotiable is what happens without it. Untreated, necrotizing fasciitis leads to widespread tissue death, sepsis and septic shock, amputation, and death. The definitive treatment is prompt surgical debridement — cutting out the dead and infected tissue — together with broad-spectrum intravenous antibiotics and intensive supportive care. Antibiotics alone cannot cure it, because the toxins have already destroyed the blood supply the drugs would need to reach the infected tissue. That is the whole logic of the emergency: the surgeon has to physically remove what the infection has killed, and the sooner that happens, the more tissue — and life — can be saved.
“It looked like cellulitis” is the beginning of these cases, not the end of them. Ordinary skin infections are enormously common and rarely an emergency — which is exactly why necrotizing fasciitis hides among them. The same line we draw between a bad outcome and malpractice runs straight through these cases: the issue is not that early necrotizing fasciitis resembles cellulitis, but whether the specific mismatch that should have raised alarm — severe pain and a sick-looking patient over a mild-looking area — was recognized and acted on.
The Standard of Care: Suspect It, Escalate It, and Cut — Fast
No one expects an emergency physician to treat every red leg as flesh-eating disease. What the standard of care asks is that clinicians know the presentation that should raise suspicion, take that suspicion seriously when it is present, and move quickly to surgical evaluation rather than settling on cellulitis and waiting. The Infectious Diseases Society of America (IDSA), in its 2014 practice guidelines for skin and soft tissue infections (Stevens DL, Bisno AL, Chambers HF, et al., Clinical Infectious Diseases), treats necrotizing infections as surgical emergencies requiring prompt operative debridement and empiric broad-spectrum antibiotics — with surgical consultation as the cornerstone of both diagnosis and treatment.
Recognizing the warning signs
The single most consistent early feature is pain out of proportion — pain far more severe than the visible skin findings would explain. A 2014 review by Goh and colleagues found this to be the most reliable early clue, and reported that necrotizing fasciitis is misdiagnosed at first presentation roughly 71 percent of the time. Other warning signs include swelling and firmness spreading beyond the area of redness, skin that is tense or shiny, and systemic toxicity — a fast heart rate, fever or a dangerously low temperature, confusion, or simply a patient who looks far sicker than a “skin infection” should make them. The classic “hard signs” — blisters (bullae), skin discoloration or blackening, a crackling feeling under the skin (crepitus), and numbness — are highly suggestive but arrive late: one study cited in the patient-safety literature found only about 43 percent of patients showed these hard signs at presentation. Waiting for them is waiting too long.
The lab-score trap
The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score, derived by Wong and colleagues in 2004, combines routine blood values — C-reactive protein, white-cell count, hemoglobin, sodium, creatinine, and glucose — into a number, with a score of 6 or higher raising concern. It can be a useful prompt, but it is dangerous when used backward, to rule the disease out. A 2016 study in the Western Journal of Emergency Medicine by Burner and colleagues found the LRINEC score had a sensitivity of only about 77 percent — meaning roughly a quarter of confirmed necrotizing fasciitis patients scored below 6 and would have been falsely reassured. The authors warned specifically against “the trap of using a ‘normal’ LRINEC score” to dismiss the diagnosis. The accepted teaching, reflected in that literature, is that a patient with high clinical suspicion goes to surgical evaluation regardless of the score.
Imaging, exploration, and the surgeon
CT or MRI can support the diagnosis by showing gas or fluid tracking along the fascia, but imaging is a supplement, not a gatekeeper — a patient sick enough to suggest necrotizing fasciitis should not wait hours for a scan before a surgeon is called. The definitive diagnostic step is surgical exploration, and the definitive treatment is debridement. That means the emergency physician’s job when suspicion is real is not only to start antibiotics but to escalate — an urgent surgical consultation, and transfer to a facility that can operate if the current one cannot. The Surgical Clock
Necrotizing fasciitis is a stopwatch diagnosis, and the evidence about timing is stark. Studies summarized in the patient-safety literature report that patients taken to surgery within 24 hours have a mortality rate in the range of 4 to 7 percent, while those whose surgery is delayed beyond 24 hours face mortality reported between 23 and 75 percent. Goh and colleagues reported that a delay of more than 24 hours to surgery was associated with a roughly 9-fold increase in the relative risk of death. Time to the operating room is, by a wide margin, the factor clinicians can most influence — and the one a malpractice review can measure most precisely.
That measurability is the point. Because the arrival time, the vital signs, the pain scores, the antibiotic orders, the surgical consult, and the incision time are all time-stamped, a reviewer can reconstruct exactly where the delay happened: whether suspicion was raised and simply not acted on, whether a surgical consult was requested and slow to arrive, or whether the patient sat on a medical floor on IV antibiotics while the infection advanced. One dangerous variant documented in the literature is the patient who presents with severe pain but no fever — in the Goh review, up to about 47 percent of patients — because the absence of fever is wrongly taken as reassurance and the workup slows down at the worst possible moment.
The Patterns of Failure
Labeled cellulitis and sent home
The most common pattern: a patient with a red, painful area is diagnosed with cellulitis, given oral antibiotics, and discharged — without the pain-out-of-proportion mismatch or the systemic warning signs being recognized as a reason to suspect something far worse.
Pain out of proportion documented but not acted on
The chart records severe pain, a rising heart rate, or a patient who “looks unwell,” but no surgical consult is called and no reassessment of the cellulitis diagnosis follows. The warning was written down, and the response stopped there.
A “normal” lab score used as reassurance
A LRINEC score below 6 — or an unremarkable single set of labs — is treated as ruling out necrotizing fasciitis in a patient whose pain and appearance should have kept the suspicion alive, exactly the pitfall the emergency-medicine literature warns against.
Antibiotics started, surgery never escalated
IV antibiotics are begun and the patient is admitted or observed, but the urgent surgical consultation that is the actual treatment is delayed for hours — while the infection continues to spread along the fascia.
Deterioration and return visits dismissed
A patient discharged with “cellulitis” returns hours later in worse pain, or calls reporting rapid worsening, and is reassured or told to keep taking the antibiotics rather than re-examined and re-worked-up as a possible necrotizing infection.
The afebrile or lower-risk patient underestimated
A patient without a fever, or one who seems otherwise healthy, is presumed to have a routine infection, and their severe pain is attributed to a strain, a bruise, or anxiety rather than investigated as possible necrotizing fasciitis.
What the Records Show
Necrotizing fasciitis cases are built on a specific, largely objective set of records, most of it time-stamped:
- The triage note and nursing assessment — when the patient arrived, the vital signs, the pain score, and how the affected area was described on arrival.
- The physician’s history and physical — whether pain out of proportion and systemic warning signs were noted, and what diagnosis was assigned.
- The vital-sign trend and repeat exams — whether a worsening heart rate, blood pressure, or pain level was recorded and whether anyone acted on the trend.
- The laboratory results and their timing — including any LRINEC components, and whether a low or “normal” value was used to downgrade concern.
- The orders and their timestamps — when antibiotics were started, when (and whether) a surgical consult was requested, and when imaging was ordered.
- The surgical consult and operative report — when the surgeon saw the patient, what was found, and the time of incision, which establish how much time passed and how far the infection had spread.
- The discharge instructions and any return-visit records — what a discharged patient was told, and what happened when they came back.
Because so much of this is generated automatically and stamped with a time, these cases often turn less on competing expert stories and more on a timeline: when the patient arrived, what warning signs were present, what was documented, and how long it took — if it happened at all — for the patient to reach a surgeon and an operating room.

- Request the complete emergency-department record from the first visit under the federal HIPAA right of access — the triage note, vital signs, the physician’s history and physical, all orders, the labs, and the discharge instructions. Do not settle for the discharge summary alone.
- Ask specifically for the laboratory results with their timestamps and any imaging reports, and for the surgical consultation note and operative report, including the time of incision.
- Preserve the records of any return visit and of the hospitalization that followed, which establish how the infection progressed and what was ultimately found.
- Write down what you remember about the symptoms and their timing — especially when the pain began and how fast it worsened — and what you were told at the first visit, while it is fresh.
- Keep the discharge paperwork from the first visit, which usually states the diagnosis the patient was given.
