This medical malpractice website is operated by The Alvarez Law Firm, 3251 Ponce De Leon Blvd, Coral Gables, FL 33134. References to “we,” “us,” or “our” mean The Alvarez Law Firm and this website.

2. What Information We Collect

We collect information you submit through the intake forms on this site. That includes:

• Your name and contact information (email address, and any other fields in the form)
• Information you choose to share about a potential legal matter
• Device and technical data automatically collected when you visit the site (IP address, browser type, pages viewed, time of visit) — collected through standard web server logs and any analytics tools we use

We do not sell your personal information to third parties. We do not use your information for marketing unrelated to legal services.

3. How We Use Your Information

We use the information you submit to:

• Review the information you send and decide whether the firm can help
• Contact you about that review request
• Refer your information to co-counsel or partner law firms that may be able to help, as disclosed in our advertising disclaimer
• Comply with legal and ethical obligations

4. Who We Share It With

We may share your information with:

• Attorneys and law firms that may take the matter (co-counsel / referral partners)
• Service providers who operate our intake technology (including Microsoft, which operates the Microsoft Forms platform used for our intake forms)
• Legal or regulatory authorities if required by law

We do not sell your information.

5. Microsoft Forms

The intake forms on this site are hosted by Microsoft Forms. When you submit a form, your data is transmitted to and stored on Microsoft's servers, subject to Microsoft's own privacy policy (available at microsoft.com). We access your submission through Microsoft's platform to evaluate your request.

6. Cookies and Tracking

This site uses:

• Google Fonts (fonts.googleapis.com) — loaded from Google's servers when you visit the site. Google may collect standard web request data. See Google's privacy policy at google.com/policies/privacy.
• Standard web server logs — your IP address, browser, and pages visited are logged automatically as part of normal website operation.
• Google Analytics — if enabled on this site, Google may collect standard usage data subject to Google's privacy policy.

We do not currently use third-party advertising trackers or behavioral ad networks.

7. California Residents — CCPA Rights

If you are a California resident, you have the right to know what personal information we have collected, delete it (subject to exceptions), correct inaccurate information, opt out of sale or sharing (we do not sell personal information), and not be discriminated against for exercising those rights.
